Double Board-Certified Psychiatric Care

Evidence-Based Psychiatric Care in Boston

Boston is at the epicenter of a medical ecosystem that could be considered to have no equal anywhere else on earth. With Harvard Medical School, Massachusetts General Hospital, McLean Hospital, Brigham and Women's Hospital, and the tightly packed Longwood medical area, Boston is the gold standard of medicine around the world. Such an ecosystem would lead one to believe access to quality psychiatric services is simple, yet it is not.

Boston has paradoxical access: Massachusetts has among the highest psychiatrist supply (>15 per 100k) but psychiatrists in Boston were the least likely to offer an appointment. Median waits are approximately 43 days for telehealth / 67 days in person, with some academic centers maintaining 6+ month waitlists.

Vital Psych MD is a psychiatry practice led by Dr. Kalvin Kapoor, a double board-certified psychiatrist from the American Board of Psychiatry and Neurology, that provides treatment in-person and through telemedicine to the greater Boston area. We offer complete assessments, attention deficit disorder diagnosis, psychopharmacology services, pharmacogenomic tests, ketamine infusion therapy, and TMS treatment where clinically appropriate.

Local Insights

The Boston Mental Health Paradox

There are factors about mental health in Boston that are genuinely contradictory. Understanding these unique regional dynamics is essential before choosing how to receive care.

01

World-Class Skill, Severe Access Shortages

While Boston ranks highest when it comes to psychiatrists, researchers, and clinical psychologists on a per capita basis, it has one of the most difficult psychiatric outpatient access problems in a large metropolitan area. It drives people to seek care through their general practitioner, with the result that many people are treated for psychiatric disorders by general practitioners during consultations for other ailments. The reputation is well deserved and the access issue is all too real.

02

Exceptional Professional Culture vs. Seeking Help

Boston favors a scientific approach, success, independence, and diligence in tackling difficult issues. This combination is perfect for MIT laboratories, MGH programs, Financial District, and biomedical engineering in Kendall Square. However, it fails to work for mental disorders. Neither depression, nor ADHD, nor anxiety, nor bipolar disorder is cured by thinking harder about it; they require clinical attention. The illusion that a person can conduct research on their condition is a widespread problem among Boston patients who postpone their visits.

03

Winter Here is a Clinical Matter, Not Weather Preference

In December, Boston gets about 4-6 hours of actual sunshine per day. Lower light, cold, and reduced outdoor activity between November and March can contribute to worsening mood and seasonal affective disorder in vulnerable individuals.

04

The "Boston Strong" Identity: Strength & Clinical Liability

Grit characterizes how the people of Boston identify themselves as tough and capable of enduring hardship without drama. While such a sense of identity is one of the factors that make Boston endure difficult times, it prolongs the time spent by people in their psychiatric state before reaching out for treatment.

Comprehensive Evaluations

Diagnostic Services in Boston

Accurate diagnosis is the foundation of effective psychiatric care. At Vital Psych MD, we dedicate necessary clinical time to understand your complete health profile.

Psychiatric Evaluation

The first visit to Vital Psych MD includes a comprehensive clinical discussion with our board-certified psychiatrist and not just an intake session or symptom checklist. Your history of symptoms, medical history, previous experience and results of any treatment, your current circumstances, and what you want to achieve are discussed. For patients from Boston who have visited overcrowded general practices or urgent care facilities for their mental conditions or have relied on over-capacity university-based counseling centers, this is usually the first time that a whole picture is put together.

ADHD Diagnosis for Adults

The educational and corporate environments of Boston tend to conceal adult ADHD. The elite institutions of higher learning, high research standards, effective coping mechanisms, and the myth that hard work is everything tend to disguise the neurological problem that needs treatment. Such people as a PhD student from MIT/Harvard who is unable to concentrate unless there is a purpose for it; a biotech researcher who excels in stressful conditions yet finds it hard to concentrate consistently; and a resident of a hospital who depends on adrenaline to perform.

Neuropsychological Testing

Neuropsychological assessment provides objective information about attention, memory, speed of processing, and executive functioning in situations when a comprehensive clinical assessment does not resolve concerns about possible co-existing conditions, ambiguous diagnoses, or cognitive difficulties that cannot be attributed to one disorder. In Boston, this approach can be particularly useful for academics or researchers where cognitive functioning is a key element of their professional life and where accurate diagnosis matters a lot.

Pharmacogenomic Testing

Pharmacogenomic testing analyzes CYP2D6/CYP2C19 variants that affect drug metabolism. Evidence shows modest clinical validity and virtually no evidence to support routine use to pick a drug. The FDA notes evidence does not support claims that tests can predict responses. We may consider it as an adjunct after multiple failed trials, not as a first-line customizer.

Evidence-Based Care

Treatment Services in Boston

From traditional medication management to innovative interventional therapies, Vital Psych MD offers advanced psychiatric treatments tailored to your unique clinical needs.

Medication Management

Medication management is an active clinical process rather than a one-time procedure. We observe the effects of the drugs you have been prescribed, fine-tune doses according to changes in your condition, and actively participate even during periods when you do not visit us. In Boston, where demand outpaces supply, most patients manage medications independently—at Vital Psych MD, we take care of you all along the way.

Ketamine Therapy

IV ketamine is used off-label for psychiatric conditions and is not FDA-approved for depression/PTSD. Esketamine (Spravato) nasal spray was FDA-approved in 2019 for treatment-resistant depression. Ketamine may produce rapid improvement in some patients but evidence and risks should be discussed individually. Its speed makes it especially relevant when facing long waitlists.

TMS Therapy

TMS is an FDA-approved, non-invasive option using magnetic fields to stimulate nerve cells involved in depression. It does not require sedation and has no systemic medication side effects, though common transient effects include headache and scalp discomfort. TMS is frequently an ideal choice for patients who have undergone multiple antidepressant trials with limited success.

Addiction Treatment

There is an epidemic of opioids taking place in Massachusetts across diverse demographics. At Vital Psych MD, we first conduct a psychiatric evaluation to determine underlying causes. It is common for patients to have coexisting diagnoses such as anxiety, depression, PTSD, undiagnosed ADHD, and insomnia among other problems.

Insomnia and Parasomnia Treatment

High-demand schedules in Boston's biotech and startup communities cause constant sleep problems. Sleep disorders impact and exacerbate other psychiatric conditions like depression and anxiety. We address insomnia and parasomnia by thoroughly diagnosing underlying causes before suggesting a treatment strategy.

Weight Loss & Obesity Treatment

Our program addresses both physical obesity and psychological factors, including emotional eating caused by mental illness, medication effects on metabolism, and body image psychology. We do not provide standalone medical weight loss; rather, we integrate it into comprehensive psychiatric care.
